Care Continuum

Care Continuum was conceived & founded in 2014 as a social enterprise, to make a significant difference to the on-demand model of Home Healthcare practised by a majority of the providers till date. We believe that health services at home require proper clinical planning and implementation to minimise the risks and prevent frequent hospitalisations. Care Continuum is therefore a much-needed step forward towards providing high quality elder care services in Kolkata.
